This template is pretty self-explanatory, and if you need examples of how it turns out, there's an ample supply of logs to look at. The only funny part is gamedate. If it's absent or malformed, the log will not show up on Logs by Gamedate. If any part of the date is unknown or undetermined, leave that portion of the field as zeros (i.e., a log that happens some time in March of 2010 might be 20100300).
